Dr. Chase is a preferred provider with Delta Dental and Cigna. However, she will also work with most insurance companies to get maximum benefits for your treatment needs, including out of pocket estimates and predetermination of benefits prior to beginning treatment. Though no benefits are fully guaranteed, we provide all information necessary to maximize the coverage for your treatment.

PAYMENTS

 

Dr. Chase accepts all major credit cards, as well as health savings plan cards. Personal checks and cash are also accepted. We also participate with CareCredit for financing long term plans.

IN OFFICE CARE PLAN

Because not everyone has coverage through dental insurance, Dr. Chase has introduced a private in office plan that offers 12 months of preventative care at a discounted price. This plan also offers discounts on all treatment and products provided in office. Details are available by calling Dr. Chase's office.
